How can we help you?

About Us

Sydney Care is a semi-independent provider, which is run and managed by an experienced and practicing Social Worker. We provide 24 hour staffed accommodation whilst assessing individual needs and risk. Promoting individual wellbeing is key and at the heart of our intervention and support. At Sydney Care, we want to ensure that the individuals who are placed with us have their individual needs met in order to prepare them to lead independent lives in the future.

Supported Living

Aims and objectives:

  • To focus on service users. We aim to provide support in ways that have positive outcomes for service users and promote their active participation.
  • To ensure that we are fit for our purpose. We examine our operations constantly to ensure that we are successfully achieving our stated aims and purposes. We welcome feedback from our service users and their friends and relatives.
  • To work for the comprehensive welfare of our service users. We aim to provide for each service user a package of care that contributes to his or her overall healthcare needs and
  • preferences. We will co-operate with other services and professionals to help to maximise each service user’s independence and to ensure as fully as possible the services user’s maximum participation in the community.
  • To meet assessed needs. Before we provide services, we ensure that a potential service user’s needs and preferences are thoroughly assessed. We aim to ensure that the care the service provides meets the assessed needs of each service user, that needs are re-assessed as frequently as necessary, and that the care and support provided have the flexibility to respond to changing needs or requirements.
  • To provide quality services. We are whole-heartedly committed to providing top-quality services and to continuous improvement in the level of care we offer.
  • To employ a quality workforce. Standards for our managers and staff are based on the national occupational standards for the care industry set by the National Training Organisation.

We provide Accommodation for persons who require Supported Living

Our Care Home Services our professionally managed homes which the person/s do not legally own or rent. We provide support to service users in a person centred approach and the individual is cared for on a day-to-day basis. We provide this service through our Supported Living Care home (care home services without nursing). Supported Living homes, enable small numbers of people to live together in a supported shared living environment. We believe in allowing people their own space to live the way they choose, take part in activities that broaden their horizons and make them happy, above all living as independently as possible.

Respite

Sydney Care also provides respite which takes place within our current Supported Living services, that provides services for people aged 18 plus with learning difficulties, mental illness and/or sensory impairment. The individual would have been formally assessed as requiring assistance to help maintain their independence.  Our values are based on the principles of inclusion, dignity, choice, privacy, independence, individuality, fulfillment and rights. We provide group-based activities, group and individual holidays and outings, 1:1 visits in the community and respite in the client’s home.

We only provide services to adults with a Learning Disability or autistic spectrum disorder as their primary diagnosis.
